Girdlestone resection arthroplasty – A limb salvage procedure after failed hip surgery in a rural setup

چکیده
Background: Excision arthroplasty of the hip was first documented in surgical practice over a century ago. However, it was made popular in 1943 by Gathorne Girdlestone who used the technique for the treatment for septic arthritis of the hip.1 Thus his name has been synonymous with the procedure, which is currently used to address problems arising from failed hip surgery including peri-prosthetic infections or recurrently dislocating prostheses. Materials & Methods: Between 2001 and 2016, the case notes and theatre charts of 57 patients who had undergone Girdle stone resection arthroplasty were reviewed retrospectively. The indications were sepsis after a hip arthroplasty in 35 cases, aseptic loosening of the prosthesis in 5, recurrent dislocation of a hip arthroplasty in 2 and failed internal fixation of a femoral neck fracture in 15. The final subjective outcome was evaluated by questionnaire performed by a single assessor. Subjective assessment was recorded in terms of pain, leg length discrepancy, functional activities, infection control, use of a walking aid, walking ability and overall satisfaction status according to Harris Hip Scoring Questionnaire. Results: After recovery from operative intervention 33% of patients experienced mild pain to no pain at all and were considered to have adequate pain control. Finally, 79.16% were happy with the outcome, 12.5% of patients were unhappy with the long-term result, and 8.33% were indifferent. Conclusions: This study found that the Girdlestone resection arthroplasty was a viable option to salvage irreversibly failed operated hip fractures in medically and economically suboptimal patients in rural setup.

Total Hip Arthroplasty in a Girdlestone Hip following a Failed Hemiarthroplasty
INTRODUCTION Girdlestone hip arthroplasty, though described as a salvage procedure for infected hip joints, can also be considered for failed Hemiarthroplasty procedures. The functional results of such Girdlestone hip may not be satisfactory. They may require total hip replacement to improve the quality of life, which are technically challenging. Here we are reporting such a case ina 60 year ol...

INTRODUCTION Girdlestone’s arthroplasty was first performed and documented by Schmalz (1817) and White (1821) to treat children with coxofemoral joint tuberculosis(1-3). In 1928, Girdlestone briefly described this procedure, using it for treating hip tuberculosis(4) and later, in 1943, Girdlestone globally disseminated this technique as a solution for treating septic and tuberculous hip patholo...

OBJECTIVE The purpose of this study is to evaluate the results and complications of hip arthroplasty with conventional stem performed as a salvage procedure after the failed treatment of an intertrochanteric hip fracture. MATERIAL AND METHODS Between 1997 and 2011, 17 patients with a mean age of 76 years were treated with hip arthroplasty for failed treatment of intertrochanteric hip fracture...
متن کاملOutcome of Girdlestone's resection arthroplasty following complications of proximal femoral fractures.
Twenty two patients who underwent a Girdlestone resection arthroplasty of the hip (pseudarthrosis coxae) following failed operative treatment for hip trauma from 1993 to 2002 were retrospectively reviewed. The indications included failed osteosynthesis of fractures of the neck of the femur (n=8), septic hemiarthroplasty (n=9), aseptic loosening of hemiarthroplasty (n=3) and recurrent dislocatio...
متن کاملExtensive subtrochanteric shortening osteotomy enables the conversion of a long lasting resection arthroplasty to a total hip replacement.
This case report presents the results of a conversion hip arthroplasty after a long lasting Girdlestone situation of 15 years in combination with an extensive shortening of the leg of 7.2 centimeters. An extensive subtrochanteric shortening osteotomy of 3.5 centimeters was necessary to restore leg length and the rotational centre of the hip. Functional outcome was excellent within the long run ...
